Shared bootstrap() replaces inline initRuntimeConfig(toRuntimeConfig(loadConfig())) in all 4 entry points. Fixes ingest command missing initRuntimeConfig entirely. Adds 32 new tests for config cache lifecycle, deep-merge, and validation edge cases.
