Add EMRS bulk migration script for Mozambique
Migrates active EMR patients to DDE Proxy via /v1/add_person.
Maps Mozambique address fields (county_district, address2, address6)
to DDE required fields. Supports resume, dry-run and patient limit.
Generates migration_mapping.csv and non-migrated CSV report.
