Feature/kabo5 nrl 1472 BaRS values

Enable Booking and Referral Service API integration by:

• Supporting "Appointment" as a new Type
• Supporting "Record artifact" as a new Category
• Supporting "Structured Document" as a new format
• Supporting fhir/json content types for attachment entries marked as Structured Documents

N.B. the scope of this work is limited to validation logic and model structure. It does not include changes to permission models or other adaptations for proxy behaviour implemented in NRL-1290.
