Split USPSRate.php

Two files, each only one class
USPSRatePackage.php
USPSRate.php

`USPSBase`

Added USPS password as optional constructor arg
Automatically adds PASSWORD=xxx attribute if present

Rewrote demos

Use Composer autoloader
Created index.php to use with php -S localhost:8888 (or whatever)
All demo scripts accept CLI args for USPS user and password, or $_GET params
