chore: add 66 new address mappings with 6-address cap#90

Map 66 unmapped location strings from the middleware API. Enforce a max of 6 addresses per mapping for broad locations (regions, timezone ranges) to keep structured data focused on the most relevant markets. Update agent, command, skill, and README with the new limit rule.
