`fields_to_phenos` didn't always recognise instanced fields correctly. This is now fixed (in the Schema "instanced" can be 1 or 2 it turns out. For getting the cancer registry fields, rather than having to manually specify the N in each array, use `fields_to_phenos`
`get_diagnoses` will now send ICD9s to `get_cancer_registry` if they are in the cancer range. `get_cancer_registry` returns an addition column "icd9" to the derived "long" cancer registry object
Not fully tested - will do some checks
And fix a character/numeric comparison
