[codex] Add federation fleet dashboard

Summary

• Add the public-facing War Room Fleet Dashboard for federated Lancelot instances.
• Add federation dashboard snapshot, stream, local detail, and approve/deny proxy APIs.
• Surface Command Center deep links, receipt-backed fleet activity, unified approval aggregation, health/attention semantics, and dashboard docs.
• Include related operator-readiness fixes for model discovery freshness, provider lane sources, Command Center quick stats, connection status semantics, auth return paths, startup hardening, and deterministic tests.

Public Safety

• Squashed from the public branch based on public/master, not from dev history.
• Scanned the staged diff for local machine paths, private dev repo references, private keys, secrets, and local-only wording.
• Feature flags remain opt-in for federation dashboard exposure.
