Closes #22
Removed excess diabetes scripts, and made scripts relating to hypertension, diabetes, and depression consistent with commenting and code structure.
I have also removed all measures (and visualisation R scripts), and will add these for depression, diabetes and hypertension in a later PR if this is required.
