Improve batched worker follow-up summaries

Summary

• replace the generic forced batched worker fallback with a meaningful summary built from worker results
• reuse single-worker forced follow-up text when available and fall back to sanitized worker summaries
• add heartbeat coverage for batched forced follow-up text
