GitHub release history still stops at v7.0.21 while npm already carries 7.0.22. This bumps the package metadata to 7.0.23, records the matching README notes, and recreates the missing minimal OpenSpec T1 scaffold so the next release can move GitHub and npm forward together from current main.
Constraint: GitHub latest published release is v7.0.21 while npm latest is 7.0.22
