Fix Trakt list refresh pagination

Change theming

• Change the color palette to be a little easier on the eyes.
• Move all of the various settings into dialogs.
• Move Add local into the Add dialog.
• Add provider icons to make it easier to see which providers are in use (and which are not)

Add title dialog

This release adds an Add title dialog. This title is now populated by Streaming Availability and adds the show or movie to your trakt playlist.

A new provider system has been added to allow selecting providers and addons that you actually pay for to help filter the lists.
