Adds a new merged "Closed At/By/Note" column to the Statement History page that combines Closed At, Closed By, and Reconciled At information into a single column. Also adds these fields to the CSV export. Controlled by the
merged_statement_history_columnsfeature flag.NU-32 | Clarify Statement History Closed On header
